STRATEGIC PLAN

Mission

2040 Partners for Health is a learning community facilitating, informing, and assessing community-based research projects, health related programs, and relevant policies through a partnership with Northeast Denver Metro Area communities (East Montclair, Greater Park Hill, Northeast Park Hill, Northwest Aurora, and Stapleton), the University of Colorado, and the Stapleton Foundation, to advance the health and healthcare of area residents and employees.

Values

The Values and Guiding Principles of 2040 Partners for Health (2040) are designed to inspire, articulate, and direct our actions as an organization.  As Board members are recruited, staff hired, strategies formed, programs developed and delivered, partnerships made, funding received, and opportunities investigated, 2040 will ensure that these values and guiding principles are supported with absolute integrity.

Courage: The belief in and commitment to doing what is in the best interests of our constituency even when faced with adversity and criticism.

Inclusiveness: The belief in and commitment to creating a climate that maximizes organizational performance by respecting and incorporating the needs and assets of diverse backgrounds, cultures, and experiences.

Collaboration: The belief that integrated resources, ideas, perspectives, and approaches between individuals, organizations, sectors, and/or communities exceeds the sum of its parts and leads to societal excellence.

Transparency: The belief in and commitment to creating an organizational environment accessible to the communities, agencies, funders, and individuals served by or who serve 2040.

Trust: The belief in and dedication to listening, learning, being accountable, and following through with our commitments in order to build trust between and among our constituents.

Impact: The belief in and commitment to ensuring that our programs and activities result in tangible improvement of health and well being among the constituents we serve.

Integrity: The belief in and commitment to having the courage, intelligence, and humility to live by the above values and guiding principles regardless of financial benefit or convenience.

2040 Partners for Health Goals and Objectives:

Goal 1:  Strengthen community participation in, and capacity to, improve health and healthcare.

Objectives:

  • Residents are trained in community leadership and research techniques and methodologies.
  • Residents play leadership roles in CBPR activities.
  • Residents train researchers to understand community needs and circumstances.
  • Residents inform and implement interventions to improve health and health care.
  • Residents learn and discuss national and local strategies for community based research and action.
  • Resident participation in health and healthcare problem solving increases by 15% each year.

 

Goal 2:  Increase awareness and use of Community Based Participatory Research (CBPR)  and action research among health focused academic and practicing institutions.

Objectives:

  • Share impact and value of CBPR with 15-20 researchers annually.
  • Change research policies among at least one academic institution to make CBPR more appealing.
  • Make 5-10 presentations to academic institutions regarding the value and method of CBPR.
  • Share results of CBPR in 2-3 national forums (i.e. conferences, publications, news releases).
  • Recruit 2-4 researchers to act as PI's for CBPR projects in the region.
  • Support researchers through training and knowledge of communities in which CBPR is taking place.

 

Goal 3: Improve health-related services, infrastructure, and policies through community-based research and interventions.

Objectives:

  • Conduct 2-4 CBPR projects that include testing interventions annually.
  • Disseminate findings from all CBPR projects to key stakeholders.
  • Meet with local healthcare providers to share findings and service improvement suggestions.
  • Meet with local elected officials to share findings and policy improvement suggestions.
  • Evaluate 1-2 healthcare providers' services (or a particular program of theirs).
  • Bring key health-related stakeholders together at least annually to discuss overall findings and develop a joint strategy to respond.
  • Document, catalog, and understand why health and healthcare improves or does not improve over the next five years.
