Heart to Heart Household Project
Heart to Heart Household Discussions is a pilot grant funded by the Colorado Clinical Translational Science Institute, given to Taking Neighborhood Health to Heart [TNH2H], a community-campus partnership, Data Review and Dissemination, a sub-committee of Taking Neighborhood Health to Heart, along with 2040 Partners for Health, a non-profit center involving departments at the University of Colorado Denver, the Stapleton Foundation, and five neighborhood groups--East Montclair, Northwest Aurora, Northeast Park Hill, Park Hill and Stapleton.
The goal of this project is to share the survey information with the community, and more importantly, hear directly from the community about health concerns and what YOU believe should be done to improve the health and well being of people in these neighborhoods. Previous Survey information focused on:
- Health Care Access and Health Status
- Food and Physical Activity
- Chronic Health Conditions
- Neighborhood Trust and Participation
- Neighborhood Safety
